Questions

Which characters get encoded?

Everything except letters, digits and - _ . ! ~ * ' ( ), matching JavaScript's encodeURIComponent.

Why does + turn into a space when decoding?

In query strings from HTML forms, + means a space. Decoding follows that convention.

Should I encode a whole URL?

Usually not. Encode each parameter value separately, otherwise the :// and ? are escaped too.
